Illness/Misadventure Form
Students in Year 10, 11 and 12 are required to complete this form when an Assessment Task has been missed. Supporting documentation (such as a medical certificate) is required to be attached. The completed form must be provided to the Curriculum Office on the student’s next day of attendance.
Application for Leave (5 days or more)
Students who require leave for more than 5 days are required to apply to the School Principal for an Application for Leave. Application Forms are located in the School Office or can be obtained by clicking above. Please be aware that the school is not permitted to backdate any Approval for Leave.
Change of Address / Family Circumstances
Use this form to notify the school of a change of address or other family information.
Consent to Use Student’s Image/Work
During the school year Joey’s staff regularly capture images of students participating in school life. These images may be used for publication in the Annual School Year Book, social media and school promotion. Consent for your child’s image to be used can be found on the Compass Parent Portal.
